Contributor Bio:  Lewis, Donald M Donald Lewis (Ph. D., Oxford University) is associate professor of church history at the University of British Columbia. He has written and edited several other books, including the two-volume Blackwell Dictionary of Evangelical Biography, 1730-1860.
