This controlled substance use log will help you keep accurate records in an easy-to-read format. Log book features a signature page to identify the initials of users and 46 separate logs to track individual bottles of controlled substances. Log pages prompt users to identify the drug, concentration, class, form, bottle number, serial number, expiration date, and size of container. Each individual entry area prompts users to enter the date, initials, initials of any additional witnesses, volume used, volume added, or volume wasted, and the total remaining volume of the bottle. An area for notes is present at the bottom of the page to explain any discrepancies.
