The Sural Fasciomyocutaneous flap is a recent modification of traditional sural flap which is harvested from the posterior aspect of upper one third of the leg. This flap becomes more reliable when a cuff of gastrocnemius muscle is dissected alongwith the sural nerve axis. This flap is extremely versatile in that is not only provides cover to the distal third of the leg, but it can also be utilized to reconstruct the whole heel, dorsum of foot as well as the middle third of leg. The size of the flap is much greater than the traditional sural flap and there is no need to sacrifice any major vessel of the limb.
